NCCCO Student Applications and Forms

The following Applications and Forms are provided to assist you with the Certification process.
National Commission for the Certification of Crane Operators NCCCO, the Standards organization, and International Assessment Institute (IAI) are the "third-parties" that develop and administer the written and practical examinations. OSTS, Inc. conducts preparation courses to assist students in preparing to take the written examinations, hosts written examinations, and organizes and offers Practical Examination sites (if needed) and Practical Examiners.

Mobile Cranes Mobile and Tower Cranes

1. Written Examination Application
(required three weeks prior to exam date. Send application to OSTS with IAI payment)

2. Physical Examination Form
2a. Instructions for Physical Exam Form
(if the operator does NOT have his/her DOT medical card, this form can be downloaded and completed by your consulting physician and submitted with your written examination application)

3. Practical Examination Application
(bring completed to your Practical Exam)

 1. Written Examination Form
(send to OSTS with IAI application three weeks prior to exam)

2. Practical Examination Form
(send to OSTS, at least, two weeks prior to practical exam)
Tower Cranes CCO Handbooks and Forms
1. Written Examination Application
(required three weeks prior to exam date. Send application to OSTS with IAI payment)

(bring completed to your Practical Exam)
